WHAT IS IT?
A program providing reduced or discounted services to any applicable patient who is UNINSURED, UNDERINSURED or HAS UNAFFORDABLE INSURANCE COPAYS.
HOW DO I QUALIFY?
Complete an Application. Provide CURRENT proof-of-income.
HOW LONG IS IT?
Once approved and a fee has been assigned, you will be active in the Sliding Fee Program for one full year at which time you may re-certify.
WHAT CAN I EXPECT?
You can expect to pay a nominal fee starting at:
$15.00 for a doctor visit
$20.00 for a dental visit
$5.00 for a visit with a Licensed Clinical Social Worker
WHAT IF I DO NOT QUALIFY?
At CHC you may STILL have the option to receive a discounted fee if you pay for your services at the time of your visit.
